DECLARATION AND CONSENT TO FREE USE OF SOUND, PHOTO AND VIDEO RECORDINGS

With reference to the audio, photo and video recordings made by SEAM engineering and/or third parties on your behalf, on the occasion of the Conference ‘Purification: acting, communicating, involving’, held on 28/10/2022 at the ComoNExT Science and Technology Park in Lomazzo (CO), I hereby

I AUTHORISE

Free of charge and without time limit, also pursuant to Articles 10 and 320 of the Civil Code and Articles 96 and 97 of Law no. 633 of 22.4.1941 – Copyright Law:

• the transmission and publication and/or dissemination in any form of audio, video or photographic material in which the undersigned appears to be represented or is otherwise recognisable

Audio-video-photographic material that affects the dignity or decorum of the persons concerned will not be processed and will be immediately deleted from the archives of SEAM engineering.

Publication/dissemination may take place:

• on SEAM engineering websites
• on SEAM engineering’s social channels;
• in print and/or in any other medium of dissemination/publication, including atypical ones;

I also authorise the storage of the aforementioned material in the computer archives of SEAM engineering and I acknowledge that the purpose of these publications is informative and popular and possibly promotional. The video-photographic material will not be used for commercial purposes.

This authorisation excludes any use for profit and transfer to third parties.

In view of the circumstance that the aforementioned material, once made public, may be subject to acquisition and republication by third parties, even without the consent of SEAM engineering or the person concerned,

I RAISE

SEAM engineering from any detrimental effect that may arise from an abusive or improper use by third parties of the aforementioned audio-photo-video material.

I CONFIRM

to have nothing to claim by reason of the foregoing and to irrevocably waive any right, action or claim arising out of the foregoing.

NOTICE PURSUANT TO ARTICLES 13 AND 14 GDPR FOR THE PROCESSING OF PERSONAL DATA CONSISTING OF AUDIO, VIDEO AND PHOTOGRAPHIC RECORDINGS

SEAM engineering hereby wishes to inform you that the ‘European Regulation 679/2016 on the protection of individuals with regard to the processing of Personal Data and on the free movement of such data’ (hereinafter ‘GDPR’), provides for the protection of individuals with regard to the processing of their personal data.

SEAM engineering, based in Via Cavour, 2 22074 Lomazzo (CO) at the ComoNExT Science and Technology Park.

SEAM engineering will process your personal data consisting of audio, photo and video recordings bearing your image and/or voice, as well as other data from which your identity can be inferred. The aforementioned data may be acquired and processed in both paper and digital form.

The personal data subject to this information notice will be published or disseminated for dissemination, information, research or scientific purposes, even after the editing of clips or individual frames, for SEAM engineering’s own purposes and in particular for publication on SEAM engineering’s website, as well as for publications, without commercial purposes, on SEAM engineering’s social channels, in print media and/or in any other media.

The personal data provided by you, including the portrait contained in the photographs/videos, will be processed in compliance with current legislation and the principles of fairness, lawfulness, transparency and confidentiality that inspire the activity of SEAM engineering. Such data will be processed both by computer and on paper and on any other suitable support, in compliance with the security measures provided by SEAM engineering.

The provision of your data is optional unless strictly related to the operation of the proposed activity or service. Failure to give consent will not permit the use of the images and/or audiovisual footage of the person concerned for the purposes indicated above.

Within the limits pertinent to the processing purposes indicated, personal data (images and audiovisual footage) may be subject to communication, publication and/or dissemination in any form on SEAM engineering websites, on SEAM engineering social channels, in print media and/or on any other media.

I, the undersigned, declare that I have read the Privacy Policy Statement above and consent to the processing of my personal data, the disclosure and publication of which I authorise within the limits described above in accordance with the GDPR. I consent to the storage of the aforementioned data for the statutory duration of SEAM engineering.

In relation to the aforementioned processing operations, you may exercise your rights under Articles 15 to 22 GDPR, such as:

• obtain confirmation of the existence or otherwise of data concerning you;
• to know the origin of the data, the logic and the purpose on which the processing is based;
• obtain the cancellation, transformation into anonymous form or blocking of any data processed in breach of the law, as well as the updating, rectification and integration of such data;
• oppose, for legitimate reasons, the processing of such data within the limits and conditions laid down.
